Employee killed by forks of trash truck

At approximately 9:55 a.m. on February 17, 2003, Employee #1 and a coworker were collecting trash using a truck with a compactor and front end forks for lifting. The coworker, the driver of the truck, got out of the cab and started to lower the forks using the control lever near the left hand side of the truck. He stopped the fork to clear some debris from the truck. He then proceeded to lower the fork and heard Employee #1 scream. He stopped the forks and ran to the front of the truck. Employee #1 was killed when he was pinned between the fork arm and the truck bumper.
