Employee Amputates Finger on Riding Floor Scrubbing Machine

On June 4, 2009, Employee #1, was a temporary worker provided by Tri-State Staffing, whose duties included sweeping, cleaning and driving the Tennant Rider Scrubber Machine. On the day of the incident, Employee #1 parked the Tennant Rider Scrubber Machine in the designated parking spot, engaged the parking brake and left the engine running while he cleaned the exterior of accumulated dirt and grease. The scrubbing brushes, which are under the main chassis, were in the down position and were on the ground. As Employee #1 came to the backside of the vehicle he observed a cable in the brush assembly under the chassis that appeared to have separated from a pulley. Employee #1 reached into the brush assembly to reattach what appeared the loose cable when the automatic shut-off feature of the vehicle caused the scrubbing brushes to retract into the stowed position, catching Employee #1's left index finger between the cable and the pulley. Employee #1 suffered an amputation of the tip of his left index finger. Employee #1 had worked for the company since August 2007.
