On July 7, 2005, Employee #1 was at a field being prepared for a carnival, performing his duties as a carnival ride operator. He was preparing the Zipper ride, which is a trailer mounted ride. The employer has an injury and illness prevention program and Employee #1 had received health and safety training. Employee #1 was preparing to wash the ride with a bucket of soap and water before the ride was set up for the weekend carnival.
