Investigation Summary
At approximately 9:00 a.m. on March 27, 2007, an employee was setting up the stamping Machine C-6 in the press area. The machine was running on the inch mode to allow the ram to come down in small increments. The single control button for the inch mode was located on the side of operator console positioned 4-inches below the press table. The employee lowered the ram down closely to the work piece by pressing the single control button with his right hand and held the work piece with his left hand on the die set-up while kneeling on the floor. The employee was observing the position of work piece when he accidentally pressed the single control button with his right hand, which brought down the ram and crushed the employee's left index finger between the work piece and the ram. The employee screamed for help and the coworkers came over to release his finger from the stamping machine. Shortly after the paramedics arrived at the site, they transported him to the Glendale Adventist Medical Center. He was referred to a hand specialist in Van Nuys, who performed the surgery to amputate a part of his left index finger on the same day. The employee was not hospitalized. The stamping machine C-6 was manufactured by Hidaka Seisakusho Co., LTD., in 1984 and its type was FLH-35 with Machine Number 59005. The bed area of press was 23.125-in. by 24-in. with 7.125-in. table extension on the front and its capacity was 35 ton. The accident was caused because the inch operating means of stamping machine were not designed to prevent the employee's hands from entering the point of operation by either requiring the concurrent use of both hands to actuate the clutch nor being the single control button was protected against accidental actuation and located that the employee could not reach into the point of operation while operating the single control.
