Investigation Summary

Employee Amputates Thumb While Transporting Window

At approximately 6 p.m. on February 5, 2007, Employee #1 was unloading double-hung windows from the trailer with three other employees at the jobsite. The windows were transported in a vertical position in the trailer and two employees were working inside the trailer who would rotate it to horizontal position and carry it out to the trailer door. Employee #1 a coworker were working outside the trailer, receiving the window from the trailer and moving it to the storage container nearby. The two employees inside the trailer placed a double-hung window on the edge of the trailer in a horizontal position and Employee #1 grabbed one end of the window and tilted it slightly to bring it out of the trailer while the coworker waited on the other side of the window to help Employee #1. When the window was tilted, the window slid open and Employee #1's left thumb was crushed between the sliding window and black stopper attached to the window frame. The dimension of double hung window was 39.625-in. by 98.875-in. and there was a locking device on top of the sliding window to prevent inadvertent movement of the window, which was not engaged at the time of accident. Employee #1 pulled his hand away from the window and noticed that his gloved left thumb was injured. Employee #1 was taken to the hospital. Employee #1 was diagnosed with a partial amputation of his left thumb, nail bed laceration and nail plate avulsion. Employee #1 was treated but was not hospitalized.
