Employee's finger amputated in conveyor sprocket chain

At approximately 3:50 p.m. on March 20, 1998, Employee #1, of Decorative Specialties Inc., was cleaning the Compu-Rip machine in Department #1. He was pulling a stuck piece of wood out of the roller conveyor when his right hand struck a sprocket chain pinch point, amputating one of his fingers. The machine was running at the time and the power supply had not been disconnected prior to cleaning. The sprocket chain drive had an inadequate guard.
