EMPLOYEE BACKS FORK LIFT OFF OF DOCK AND IS CRUSHED

At approximately 4:00 p.m. on April 12, 1998, Employee #1 was driving a Nissan fork-lift Number 7 in the dock area to unload material from a trailer truck. He set a pallet down and tried to drive the fork-lift in reverse and drove off the dock. He stated that he tried to stop the fork-lift but he could not. The fork-lift fell on his left leg which resulted in leg, rib, and pelvic fractures, and a back injury. He was hospitalized at USC Medical Center for two months. Employee #1 stated that he was not a certified operator and had no training in fork-lift operations. The fork-lift was inspected by a representative of Crown for-lift trucks on April 13, 1998 and no defects were found except for some damage to the body and electrical wiring.
