Employee's fingers amputated in flour blender

At approximately 5:15 a.m. on June 5, 1998, Employee #1, of Vons Companies, Inc., was cleaning a flour feeder machine in the mixing room when his left hand struck the moving rotary blender, amputating three of his fingers. There were no witnesses to the accident. Apparently, the lamps on the power control unit of the machine were not on, which led Employee #1 to believe that the power had been turned off.
