Employee injured when caught by storage rack

Employee #1 inadvertently drove his forklift too close to a storage rack in a freezer and caught his leg. Employee #1 sustained a severe laceration 6 in. long, 3 in. wide, and 0.75 in. deep. Employee #1, was hospitalized, and required surgery.
