Employee injured when pallet wrapper amputated finger tip

At 8:30 a.m. on Tuesday morning, April 20, 1999, Employee #1 was operating the double wrapper on turntable A. He was wrapping a pallet of 25 pound bags, 100 bags that weighed approximately 2,500 lbs. Employee #1 had already placed a pallet cover over the pallet of bags and had attached the stretch wrap to the pallet. He proceeded to wrap the pallet by turning the power on, causing the turntable to start to turn. Employee #1 noticed that the stretch wrap was draping down and starting to work its way underneath the turntable. Employee #1 attempted to unjam it without first stopping the equipment. Employee #1 sustained an amputation of his right middle finger up to the first joint, and was hospitalized.
