Employee's finger amputated on poultry netting machine

At approximately 8:45 a.m. on March 29, 1999, at a wire product manufacturing facility, Employee #1 and Employee #2 were installing a sprocket and adjusting the chain of a poultry netting machine. Employee #1 was in a manlift, working on top with the chain and sprocket. Employee #2, the foreman, was below with the controller and working in the lower level of the poultry netting machine. The machine was not locked out because the employees needed to observe the machine and adjust the chain accordingly. Employee #1 informed Employee #2 that the chain was ok. Employee #2 then told Employee #1 to come down from his location. Employee #2 then began to work again under the machine at the lower level without contact with Employee #1. Employee #1 then took a second look at the chain and decided that something did not look right with the chain and began to check the tension on it with his finger. At this time Employee #2 jogged the machine which amputated the left middle finger of Employee #1 at 3mm distal to the tip. Employee #1 was transported to a clinic to receive treatment. Extension tools were not provided or required nor was training provided for extension tools.
