Employee's femur fractured in cardboard machine

At about 5:45 a.m. on October 21, 1998, an employee of the Crockett Container Corporation was acting as a helper on the F-1 50 Langston Flexo Folder and Gluer (Serial Number 772520). This was not his normal assigned machine and he had operated this particular machine only ten times during the previous year. The Langston Flexo folds and glues cardboard sections in preparation to make containers. In order for the machine to operate, three separate keys held by three separate employees must be inserted into the machine prior to startup. During the shift, a jam developed in the Langston Flexo machine. The assistant operator opened the section of the machine where the jam was located and he, along with the primary operator and the stacking employee, removed the three keys required to operate the machine. The assistant operator then cleared the jam and all three keys were reinstalled. The assistant operator went to the rear of the machine to restart and close the machine section that was opened when Employee #1 noticed some scrap cardboard in section about to be closed. As he reached in to clear the cardboard, the machine section doors closed onto the employee and fractured his right femur. He was transported to Whittier Presbyterian Hospital, where he was admitted for ten days.
