Employee's fingers amputated in press brake

At approximately 9:00 a.m. on May 21, 1997, Employee #1, of Durfee Sheet Metal Works in El Monte, CA, was working on a Chicago Dreis and Krump press brake. He was pressing a 10 by 3 metal screen between a folded 10 by 1 metal strip to be used in building a cage for an air compressor. He was reaching to pull the part from the press brake when the fingers of his right hand became caught in the point of operation and were amputated. The machine's presence-sensor device was not working at the time of the accident. The employer was cited for a serious violation of T8CCR 4206(a). Additional violations were also found, and the employer was cited accordingly.
