Employee injures leg when pinned under chiller head

At 7:30 a.m., at Abm Engineering Services, Employee #1 was at a loading dock surveying a front and back circular chiller head that he and Employee #2 had removed for annual maintenance. The heavier front head was laid horizontally on the floor, while the back head was standing on its side on 2-in. by 4-in. wood dunnage held upright by a chain hung from the forks of a forklift. The back head was steel and weighed about 3,200 lb and was about 3 ft in diameter and 5 in. thick. As Employee #1 approached the back head, the chain came off the fork and the back head rolled toward him, dropping on his leg. He was trapped under the head for about 6 to 10 minutes. He was hospitalized for an exploded knee.
