Investigation Summary
At approximately 4:45 p.m. on November 17, 1999, Employee #1 was cleaning metal fragments from a roll former machine. He reached into the machine while it was operating to extract the metal fragments from the in-running roller of the machine. As he reached into the machine, the in-running roller caught his left index finger at a nip point and pulled it into the machine. He sustained an amputation of the left index distal phalanx from the force of the roller.
