Investigation Summary
At 11:00 a.m. on April 20, 1999, Employee #1 was kneeling on the floor in front of the acid storage closet while holding a bottle containing sulfuric acid. The bottle slipped from her hands, dropped and broke on the vinyl floor causing the acid to spill. She slipped on the acid-wet floor, falling face down, causing acid burns on her face, chest, forearms and knees. She was hospitalized for treatment of her injuries.
