Conveyor Amputates Employee's Arm

At approximately 9:30 a.m. on August 11, 2003, Employee #1 was cleaning the tail end of a conveyor on an Allero Baler. This machine bales the sheet plastic which is taken off the strawberry fields for recycling. The belt conveyor takes the plastic up to a hopper for the baler. The tail end of the 41-in. wide belt conveyor is at ground level and needed to be cleaned of the plastic in the area. Employee #1 hit the emergency button on the side of the conveyor and the conveyor stopped. Employee #1 was cleaning the conveyor with his left arm in the arm of the tail roller when the conveyor started and amputated his left arm between the elbow and wrist. Lockout or tag out procedure was not used. Employee #1 was hospitalized.
