Investigation Summary

Investigation Nr: 201096641
Event: 07/18/2003
Employee suffered concussion after slip and fall from truck

At approximately 8:30 p.m. on July 18, 2003, Employee #1, a truck driver, was helping to unload a trailer of grocery items at Albertson's Grocery Store at 1120 East Clark Avenue in Ocutt, California. The trailer had been loaded clear to the back with only about a foot left. The load was two pallets wide, one pallet was taken off and they were starting to unload the second pallet. One of the pallets had water bottles and due to the loading or in transit, one or more of the water bottles broke and at least one came loose from the pallet. There was water on the floor of the trailer and at least one bottle. When the second pallet was being moved the employee moved out of the way and slipped on the floor and stepped on the loose bottle. The employee fell out of the trailer about 4 ft to the black top pavement. Employee #1 was hospitalized. There was no light to see what was on the floor.
