Event: 12/05/2000
An employee amputated his left finger

On December 5, 2000, an employee, hired as a general helper, started working in building 14, along with three other employees as directed by the second shift supervisor. With a 15 ton overhead crane, they placed the 8 ft by 15 ft sheet metal in position over the center die. The employees inserted the material's edge into the north (left) and south (right) jaws. The jaws were closed to hold the material in place. The position of the sheet metal did not match the die. The jaws were opened to be realigned. On all the sides the shims were fine except the North-West side. The employee from the north-east position came to the north-west position to assist his co- worker. The material was 0.125 inch short of that jaw section. While his left hand which was placed partially inside the open jaw, gave the operator signal to close the gripper jaws on the north side. As the operator pushed the button, the jaw sections started to close from middle to both ends and closed onto his finger. The operator opened the jaw when he realized the employee finger was inside and pulled his hand out of the danger zone. He was hospitalized for his injuries.
