Investigation Summary

Investigation Nr: 201054418
Event: 08/11/2000
Employee's finger amputated on table saw

At approximately 8:05 a.m. on August 11, 2000, Employee #1, a machine operator for Wilson Partitions, was cutting a scrap piece of block wood down to 2 1/2 in. by 22 in. for use in a double miter saw. As he was working, the piece of wood made a sudden movement and his left hand struck against the saw blade. Employee #1 suffered traumatic partial amputation of his left little finger, He was transported by ambulance to White Memorial Hospital, where surgery to reattach his little finger was unsuccessful. Employee #1 was inexperienced as a machine operator and had not been instructed on the hazards associated with the table saw. The saw guards were fixed into position by a piece of tape, which prevented them from retracting and functioning properly to provide optimum protection.
