Investigation Summary

Investigation Nr: 201054475
Event: 08/19/2000
Employee injured in fall through skylight

At 2:00 p.m. on August 19, 2000, Employee #1, working for a roofing contractor, was working up on a flat roof approximately 18-feet above ground. Employee #1 says that he was assisting his foreman by holding the hose as the foremen was spraying a foam coating onto the roof. Employee #1 noticed that the hose had wrapped around a skylight and began pulling the hose. For unknown reason the hose pulled Employee #1 in the direction of the skylight and he fell through. The skylights were clearly marked with warning lines. Employee #1 suffered a fractured back, left wrist, and a deep laceration to his head. He was taken to Huntington Memorial Hospital by ambulance where surgery was performed to his wrist. Employee #1 was hospitalized for four days.
