Investigation Summary
At approximately 5:50 a.m. on April 18, 2001, Employee #1 of J and J Snack Foods Corporation was cleaning a churro machine. The sleeve of his lab coat got caught by a moving chain belt and sprocket causing his right arm to be pulled into the machine. Employee #1 sustained a compound fracture to his left arm and was hospitalized.
