Employee fractures hip when step ladder collapses

At approximately 5:30 p.m. on March 22, 2002, Employee #1 was carrying a 130 pound valve up an 8 ft fiberglass step ladder marked Facilities BA12. He was carrying it to an elevation of 11 ft under a ceiling to install it into the water plumbing system. A coworker was sitting on the top of a metal deck 11 ft high waiting for Employee #1 to bring up the valve. As he was climbing the step ladder, he was holding the valve on his right shoulder with his right hand and holding the ladder with his left hand. When he was on the third or fourth step from the top of the ladder, the ladder collapsed. He fell and fractured his right hip and was hospitalized.
