Investigation Summary
At approximately 1:00 p.m. on March 27, 1996, Employee #1 was working on the set of the television show "NYPD Blue" at the Fox lot. Employee #1 and a coworker were climbing over the stairway railing at the Mulberry Street cop bar set, when Employee #1 slipped on the slick steps and fell 15 ft onto the concrete floor. He suffered a broken thumb and a compressed disc.
