Investigation Summary
At approximately 11:40 a.m. on May 8, 2007, Employee #1, a relief machine operator, was cleaning a dough cutting machine called a Model K, Series Number 900k. The cylinder of the machine has 4 pistons, which pulls the dough and cuts it by a wire. They were doing a change-over cleaning as the machine production was going from wheat to white bread. Employee #1 put her hand in the hopper directly above the cylinder and scraped out the remaining dough without realizing the machine was operating. She apparently pushed the wrong button to turn off the machine, and had not lockout the machine. The tip of her right middle finger was amputated.
