Investigation Summary
At 12:09 p.m. on February 11, 2004, Employee #1, a laborer of an automatic carwash station, was washing a car. The car wash system was moved by a conveyor system. Hanna Equipment made the conveyor system with Model Number BO1 and Serial Number 668. Several sets of rolls were attached to the conveyor chain that moved around the floor of the bay to move a car in the carwash system. At the tail end of the conveyor system, the rolls went under the floor into a hole to make a 180-degree turn. After making the turn, the rolls came out of the hole by lifting up the floor plate. After started the conveyor system, Employee #1 tried to walk across the conveyor system. The floor plate was in open position. His foot slipped into the hole, and his ankle was entrapped between moving rolls and a metal structure inside the hole. Employee #1 lacerated his right ankle and was hospitalized.
