Investigation Summary

Investigation Nr: 201162468
Event: 03/26/2001
Employee Loses Finger As He Connected Trailer To A Truck

At 6:36 p.m. on March 26, 2001, Employee #1 lost his right index finger while attempting to connect a power winch trailer onto the hitch of a truck so that it could be transported to the work site. His fingers were crushed between the frame and the front wheel bracket of the trailer. His employer, SECC Cable/Communications, is a company involved in the installation of cable communication. The power winch trailer is also known as a "Hogg Davis", because it was made by Hogg and Davis Incorporated. Its purpose is to transport cable and reel in cable. The Model Number was H100BEPTU. The Serial Number was 363355REB375. The power winch trailer contains an on-board internal combustion engine and electronic controls to power the winch motor, maneuver the trailer, and lower or raise the trailer onto the hitch of trucks. The power winch trailer contains two wheels in the rear and one wheel in the front that can be steered mechanically for fine adjustment using a moment arm pole that detaches. On the day of the accident, Employee #1 and coworker began the workday as usual by taking work orders from a customer and loading the power winch trailer to the stake-bed truck. His coworker was at the controls of the power winch located on the front and side of the power winch while Employee #1 was directing and fine adjusting the front wheel of the power winch. When the power winch trailer and the stake-bed truck hitches were aligned, his coworker began to lower the power winch trailer onto the hitch. Employee #1's concentration was consumed aligning the trailer to the hitch and make proper adjustments as the trailer was being lowered. He apparently was not paying attention as to the location of his hand. Employee's right index finger was amputated when his finger was crushed between the frame and the front wheel bracket of the trailer. His coworker was the only direct witness to the incident. Employee #1 was transported to Little Company of Mary Hospital for treatment.
