Investigation Summary
At approximately 8:20 p.m. on July 16, 2002, an employee was performing his duties of employment as a warehouseman with Reliance Steel. Reliance Steel distributes fabricated sheet metal utilizing various types of equipments including a keystone Roller Leveler built by Herr-Voss. The keystone Roller Leveler is a conveyor system intended for applications where precision flattening or leveling of sheet metal stock is required. Packing the sheet metal according to customer's specifications is part of the routine operation at Reliance Steel. The employee's primary duties included bundling the sheet metal with metal straps which he had been doing for about eight months. At the time of the accident, the employee was working along with another coworkers at keystone Roller Leveler where he was attempting to put three straps on to a stack of sheet metal measured 36-in. by 60-in. The total weight of this stack was estimated about 3,000 pounds. The stack was placed on the leveler so it can be bundled and further transported to the customer. It was suspected that the coworker did not see the employee as his view might have been blocked by the stack of metal. The coworker activated the run button from the control panel of the leveler. The employee, who had both his legs in between the running roller, was in standing position when the stack moved forward and pinned his left leg against the roller. The employee then fell backward and landed on the conveyor system head first. It was determined to be a common work practice to stand inside of the conveyor system so the stack can be properly packed. When the dimension of the sheet metal was less than 96-in. in length or if customers requested the stack to be strapped in a certain way, the employees must do their work from standing inside of the conveyor system in order to reach the materials. The employee was taken to White Memorial Hospital by the paramedic where he was hospitalized and treated for his injury.
