Investigation Summary

Investigation Nr: 126081488
Event: 01/02/2001
Employee injured when hand crushed by loading arm

At 12:20 p.m. on January 2, 2001, Employee #1, a worker for the City of Claremont, was seriously injured while standing on the side of a trash truck in operation by a driver. Employee #1 had worked for the City of Claremont since May 19, 2000. Employee #1 worked as a swamper to watch out for the driver of a trash truck and to assist the driver by positioning the trash bins to be picked up. Employee #1 was standing on the side of the trash truck while the driver was lifting the loading arm and fork in preparation to drive to the next stop to pick up a trash bin. During the investigation, it was found that the injured and other employees were not trained nor instructed with respect to the safety hazards associated with riding on the step of a trash truck in motion and with riding on the step when the loading arm and fork were in motion. In practice, most of the swampers were riding on the step of the trash collecting truck while the truck moved from one trash bin to another trash bin. Grab handles were not provided for the employees riding on the step of the trash truck. Employee #1's left hand was placed on the arm of the front loader truck. While distracted by a lady yelling from the Boardwalk Apartment, the ring finger and the little finger of his left hand were crushed between the loading arm and the fork of the trash truck. Employee #1 was hospitalized.
