Investigation Summary
At about 12:30 a.m. on June 15, 2000, Employee #1 was inflating a large tire employee #1 was a recent new hire and was not experienced in repairing large truck/bus tires. Employee #1 said he had received about 2 days of training from employee #2 on how to repair and inflate tires. Employee #1 received a serious occupational injury while he was inflating a single piece Goodyear metro miler bus tire, model b 305/85r/225. This tire had between 90 and 100 psi, when the accident occurred. Employee #1 stated that he had placed the tire and rim on top of a 5 gallon bucket with the outside rim facing up. Employee #1 was having trouble getting the tire bead to seat, so, he applied more tire lube and turned the tire over so the outside rim was facing down. The tire bead seated. Employee #1said he then put the valve core in the tire. Employee #1 then leaned the tire against some other tires and began to inflate it. When employee #1 had about 90 psi in the tire, he removed the air chuck he was using because its gage only went up to 90 psi. He used a separate gauge to check the tire pressure and continued to inflate the tire, but it was taking a long time so he sat down on a 5 gallon bucket. Employee #1 said just as he took the air valve off of the valve stem he heard a pop and felt pain in his left leg and debris in his eyes. Employee #1 said that when he was being trained, employee #2 did mention the tire cage but did not say it was necessary to use it. Employee #2 did not use the tire cage when he was demonstrating to employee #1 how to repair, and inflate tires. Employee #1 was transported to San Gabriel Medical Center, where he spent at least five days to treat the following injuries: a severely swollen left knee, slight fracture of the 3rd and 4th fingers of the right hand, broken left wrist and forearm, and a concussion. Employee #1 was hospitalized with fracture.
