Investigation Summary
At approximately 4:30 p.m. on August 9, 2000, Employee #1's left hand was crushed by a dryer blade. He suffered a laceration and loss of feeling and nerve damage to the first and middle fingers of his left hand. Employee #1 and two coworkers work for Teknor Apex, a plastic pellet manufacturer in Industry, California. The two coworkers did not see the incident but were in the vicinity of the incident. The injuries were reported to the division by the employer on August 11, 2000. Employees #1 and the two coworkers were part of the oncoming shift. The two coworkers were preparing to clean the Number 2 compound line. Employee #1 and one of the coworkers were on the upper level. The other coworker was on a different section of the upper level, about 50 yards away. Employee #1 was about to clean the dryer and another coworker was about to clean the shaker. Employee #1 said that when the off going shift finished a run and the on coming shift was going to clean the machine, the off going shift always turned everything off. So, he did not check the control panel down below nor the disconnect switch on the upper level. The employees, when asked, also stated that they rarely, if ever, locked out the equipment. Employee #1 failed to secure the power and to lock out the dryer disconnect prior to removing the cover and screen in order to clean the dryer. When employee #1 reached his left hand into the dryer to scrape away the debris his left hand was smashed between the dryer blades and the side of the dryer enclosure. His coworker was working about 10 feet away cleaning the shaker, but did not see the accident. His coworker helped Employee #1 down the stairs after the incident and notified the other coworker about the incident. Then, one of the coworkers turned off the dryer. Employee #1 was transported to Queen of the Valley Hospital. He spent four days in the hospital for treatments for deep laceration and loss of feeling and nerve damage to the first and middle fingers of his left hand.
