Employee injures arm in fall from ladder

On April 3, 2004, Employee #1, a refrigeration mechanic working at the Vons Grocery distribution warehouse, was repairing a bay door while standing on a ladder. The pry bar he was using slipped, causing him to lose his balance and fall approximately 5 feet to the ground. He suffered a fractured left arm and was hospitalized.
