At 8:00 p.m. on July 27, 1999, Employee #1 was repairing a ML Strapper machine. The safety guard was off and the electrical power was on. Employee #1 was making repairs when right hand contacted the moving chain and sprocket. The third and fourth digit fingertips were amputated.
