Investigation Summary
On August 29, 2007, Employee #1, a maintenance mechanic, and a coworker had pumped out the oil from a 28,000 TP pit and put it into 55-gallon drums in the basement. They used a Sandpiper pump with compressed air supplied through 0.75-in. air hoses. The air pressure from the main valve was 110 psi and the maximum pressure of pump was 100 psi. At approximately midnight, they completed the job and were cleaning up the area. The coworker was cleaning the air hose on the suction side of pump and Employee #1 was cleaning the air hoses on the supply side. This consisted of two air hoses in-line. Employee #1 first disconnected the air hose from the pump, cleaned and wrapped it. He then disconnected two air hoses without shutting down the air supply. As he released the quick disconnect one of the hoses slipped from his hand and struck his thigh. Employee #1 did not feel any pain so he continued to clean the hose and finished his shift. He noticed a small mark on his right thigh. For next couple of days, the mark on his right thigh got redder and longer and by Saturday night he was experiencing a fever. Employee #1 was hospitalized for 10 days for cellulites.
