Employee injured when leg amputated by forklift

At 8:40 a.m. on June 4, 1999, Employee #1 was unloading a trailer using a forklift. Employee #1 parked his forklift on a ramp leading to a truck which he was unloading. The ramp was inclined down into the truck. Employee #1 said he then set the parking brake and got off the forklift to cut the bands on the material being unloaded. While standing in front of the forklift, the forklift rolled toward him and the forklift blades hit his lower right leg. The contact created a crushing injury to the lower right leg and the leg was amputated below the knee. Employee #1 was hospitalized.
