Investigation Summary
On June 11, 1999, an employee was backing up a stand-up type forklift. The employee stated that he removed his left foot from the pedal that should stop movement of the forklift. He stated that the forklift continued to coast and he pumped the pedal. His left foot moved outside the running lines of the forklift and his foot was crushed between the forklift and a pole. This created a crushing injury to the left foot and the foot was amputated.
