Investigation Nr: 201052529
Event: 08/19/1999
Employee Falls Through Skylight 26 Feet and is Hospitalized

At 8:35 a.m. on August 19, 1999, Employee #1 was attempting to clear jammed corrugated waste scrap material from a cyclone. The cyclone is used to collect corrugated scrap for eventual bailing. The cyclone jammed, causing pieces of corrugated cardboard scrap to be ejected onto the rooftop. Plastic bags were used as a means of collecting the scrap scattered about the rooftop. A bag containing cardboard scrap had been placed on top of an acrylic skylight cover. As Employee #1 attempted to retrieve the bag his foot slipped resulting in him falling through the skylight cover. Employee #1 fell 26 feet onto stacked corrugated cardboard sheets and then four feet onto the concrete floor. Employee #1 was hospitalized with bruises, contusions and abrasions.
