Employee's fingers amputated when caught in machine

At approximately 10:00 p.m. on August 10, 1999, Employee #1 was working on press Number 40, Serial Number 1197 when she noticed a big ball of dried ink on the lower part of the frame. She grabbed a rag to clean it and the toggle connector caught the rag and pulled her fingers into the point of operation. Her middle finger had multiple fractures, and the index finger was amputated at the tip and the bone was partially chipped off. She was hospitalized.
