Employee's Fingers Are Crushed in Packaging Machine

At approximately 7:30 a.m. on November 24, 2006, Employee #1 was a leadperson, working at a meat packing facility. Production had stopped on the Multivac Number 2 packaging machine, Model Number 855D, Serial Number 330-012, due to a jam of packaging film in the feed section. To clear the jam, Employee #1 used her left hand to reach under the machine guard, into the feed section. She had not deenergized the machine. Employee #1 was injured when the machine began a production cycle and her left hand was caught in the tray forming die. The fingertips on the index, middle, and ring fingers of her left hand were crushed. She was treated at the Western Hand Center for amputation of her injured fingers. She was not hospitalized.
