Employee's Hand Crushed In Mill Machine

At approximately 7:30 a.m. on June 15, 2004, Employee #1, a mill man, was working alone on the employer's mill #1 preparing the mill for the day's operations and was attempting to clean grease from the side of the rollers. Employee #1 was standing behind the mill and momentarily placed the machine in reverse. Before the rollers came to a complete stop, Employee #1 reached with a rag in his right hand beneath the roll to clean the grease and his index finger and thumb were caught and crushed between the rolls. Employee #1's injuries resulted in the amputation of his right index finger and the tip of his right thumb below the nail bed. Employee #1 had received lock-out tag-out training, including specific training on how to lock-out the mills before performing cleaning operations. Employee #1 was hospitalized.
