Investigation Summary

On June 12, 2001, Employee #1 and two coworkers were moving wall elements that were part of a set for a movie. The elements were made of wood and resembled wood stud walls in a home, except they were lightweight. The units were on rolling dollies to move them into place. When the elements were in the vicinity where they were to be positioned, they were removed from the dollies and set on the floor. Employee #1 and one coworker were lifting the unit to allow the other coworker to remove the dollie. Employee #1 was counting to three to lift and place the element on the floor. When Employee #1 did so, he let the unit down on his own finger, while the coworker's end was still about 4 in. off the ground. As the unit went down on Employee #1's finger he jerked it out and, as he did, it peeled the tissue off the tip of the middle finger of his right hand. Employee #1 was hospitalized.
