Employee's fingers amputated by a milling machine

Employee #1, a wood milling machine operator, was cutting wood on a Profimat 4 Side Milling Machine, which smoothens surfaces of all four sides of a piece of wood in one pass. It has four blades that are guarded by a hood with appropriate interlocks. The end cutter is 5.5 in. from a guard that is 1.875 in. from the bed. A piece of wood became stuck in the machine and Employee #1 reached under the guard and contacted the cutter amputating part of his thumb and forefinger. To assure a safe operation of the milling machine the guard should be 16 to 18 in. from the blades.
