Event: 05/10/2004
Employee Is Injured in Fall from Ladder

On May 10, 2004, Employee #1, an electrician, was working as a contract employee in a school to rewire its woodshop. As Employee #1 was standing on an extension ladder with his feet about 6 to 7 feet from the floor, the ladder slipped and Employee #1 fell. Employee #1 was taken to a nearby hospital, where he was hospitalized and treated for fractures on both his legs and arms. The accident investigation revealed that the woodshop had a polished concrete floor and the ladder was not tied-off or restrained from slipping.
