Investigation Summary

Employee #1 and Employee #2 were in a Genie brand, Model S-85, 4WD, 1999 model year, Serial Number 1696, 500-pound capacity boom lift. Employee #1 was operating the lift and Employee #2, a fireproofing inspector, was inspecting fireproofing that had been applied to steel beams. The beams being inspected were in a row that was 8 ft inside an outer row. The outer row had 4-in. by 10-in. steel plates welded to them. The lift ascended to the inside beam and then translated laterally until the boom was flat up against the outer beam, but above the attachment plates. Employee #1 attempted to lower the boom while it was still against the outer beam and above the horizontal plate. From his vantage point he couldn't see that the boom was resting on the plate. As he continued to try and lower the boom, the wheels on the side of the lift closest to the basket started to come off the ground. At some point the boom arm came off the horizontal plate and the basket fell approximately 6.67 ft coming to rest on a horizontal beam. Both employees were wearing fall protection harnesses and hard hats. Employee #1 fractured his neck and Employee #2 received a head injury and died. Both employees were properly trained and the equipment was properly maintained.
