Employee's Thumb Amputated by Table Saw

At approximately 6:00 a.m. on July 11, 2002, Employee #1, a prop maker, was using a Powermatic 10-in. table saw Serial Number 9466335, with a Biesmeyer self adjusting blade guard. Employee #1 was cutting a piece of medium density fiberboard. Employee #1 was not using a push stick at the time. Medium density fiberboard is slippery and this piece was covered with fine sawdust. Employee #1 had his hand on the left side fall-away. Employee #1's hand slipped and slid under the guard. Employee #1 suffered the amputation of the tip of his left thumb. Employee #1 was hospitalized.
