Investigation Summary

Investigation Nr: 201096286
Event: 02/07/2003
Three employees inhale black smoke

At approximately 4:50 p.m. on February 7, 2003, Employees #1, #2, and #3 were near a HT 500 Van Dorn plastic injection molding machine (number 413) when it released black dust. The material being used was corzan and according to the manufacturer, the material will degrade and build pressure until it is released in black dust when it is over heated in one of several ways. One of the employees was operating the machine and had taken a ten minute break. When he returned the machine nozzle had been plugged. This is one of the ways heat and pressure are built up according to the manufacturer of the material and that can cause the release of black dust. The employees inhaled this dust and were taken to the hospital where they were treated and released.
