Investigation Summary
On January 24, 2002, Employee #1, a master craftsman, was setting up feed gears of machine Number 3, an Acme-Gridley, 6-spindle chucking machine with Serial Number 71632-A. Machine Number 1-B was running the exact same job in that department. The Model Number was RAC-6, and the Serial Number was 71676-B. Employee #1 wanted to check the numbers on the feed gears that were utilized on machine 1-B to be exactly sure. Employee #1 went to machine 1-B, opened the safety cover of the feed change gears and noted the number of the forward gear. Since it was hard to see the numbers on the rear gear due to machine oil and grease, Employee #1 used a rag and wrapped that around his right thumb to wipe off the oil from the gear to see the numbers on it better. The rag became caught in the running gears and pulled his thumb into the machine, amputating his right thumb. This equipment was capable for more machining operations in a single setup to finish product to high standards of sustained accuracy in the shortest time. His employer failed to ensure that the machine had proper safety guards in place. Employee #1 was hospitalized to treat his thumb.
