Investigation Summary
At approximately 7:45 a.m. on October 17, 1997, Employee #1, a structural iron welder, slipped from the beam chair from which was working. The employee was wearing a harness with a 6-foot lanyard. The 6-foot lanyard was tied to a 6-foot cable, thus increasing the length to 12 feet. Employee #1 slipped and fell backwards and hit a perimeter fall protection cable with his chest.
