Investigation Summary
On July 2, 1999, Employee #1, a laborer at a facility that makes cement blocks, died and also fractured his left hand when he stuck his hand through a mast. Using a fork lift, Employee #1 emptied a scrap bin loaded with a broken product. As he was lowering the forks, the forks became stuck. The forks would not go to a lower level. Upon inspection, he discovered that a broken piece of block was stuck in the chain drive of the fork lifting mechanism. He decided to climb up on the back side of the mast to stick his hands through the mast to dislodge the rock. While dislodging the rock, the forks fell crushing his left hand. As a result, Employee #1 died and also fractured his left hand.
