Investigation Summary
At approximately 9:00 p.m. on August 1, 1996, Employee #1 was using a rag to clean a right angle gluing machine while it was in operation. His right hand became caught in the machinery and he sustained crushing injuries and loss of blood. Employee #1 was in charge of the machine's operation and maintenance, and he had a history of cleaning it while it was running. In 1989, one of his fingers was partially amputated while he was cleaning the machine as it was operating.
